About the Commissioning Toolkit
Asthma UK, the British Heart Foundation and Diabetes UK have teamed up to produce this toolkit, which provides advice and support on all aspects of commissioning for long-term conditions, as well as national guidance, policies and case studies on best practice for commissioners across the UK.
Asthma, heart disease and diabetes affect millions of patients within the United Kingdom, but the services for long-term conditions commissioned on behalf of these people are not always well-matched to their needs.
As a partnership of patient groups, we are working to help ensure that patients are involved in the vital processes that determine the services that they receive. This has not historically been the case, so we are working with commissioners and expert organisations to provide good examples on how to effectively engage the public in their work.
Asthma UK
Asthma UK is the charity dedicated to improving the health and well-being of the 5.2 million people in the UK whose lives are affected by asthma. We work together with people with asthma, health professionals and researchers to develop and share expertise to help people increase their understanding and reduce the effect of asthma on their lives. Our vision is 'control over asthma today, freedom from asthma tomorrow'.
Diabetes UK
Diabetes UK is the largest organisation in the UK working for people with diabetes, funding research, campaigning and helping people live with the condition. Our mission is to improve the lives of people with diabetes and to work towards a future without diabetes.
British Heart Foundation
The aim of the British Heart Foundation is to play a leading role in the fight against disease of the heart and circulation so that it is no longer a major cause of disability and premature death. The BHF is the largest independent funder of heart research in the UK. We play an important role in funding education, reaching the public and health professionals. And we provide life-saving cardiac equipment and support for rehabilitation and patient care.
National Services for Health Improvement
National Services for Health Improvement are a unique service provider to the fast changing world of the NHS. NSHI are shaped to offer support services to Strategic Health Authorities, Secondary Care and Primary Care Trusts.
In addition to offering consultancy services for service redesign NSHI operate specialist clinical services offering a wide range of support. We pride ourselves in offering highly qualified nursing staff that has a vast knowledge in Long Term Disease Management who can provide nurse led clinics offering support for QoF or indeed our IT staff who support general practice in identifying and reviewing specific patient groups.
In addition to all of the above NSHI have an reputable medical publishing department which focuses on commissioning titles to help assist in educating various healthcare professionals and practice managers to deliver improved patient outcomes within budget.
